Kristin Lincoln-Ellzy serves as a Continuing Care Coordinator at Saint John Vianney Center. In this role, she coordinates post-discharge planning and reintegration working closely with residents and their diocesan or congregational leadership to guide the development of individualized Wellness Plans designed to support a coordinated transition and continuity of care. Kristin joined the Center in 2014 and brings more than 20 years of experience in the behavioral health and addictions field. Her professional background includes work within the juvenile and adult justice systems, drug courts, state health and human services agencies, and inpatient treatment programs. Prior to joining Saint John Vianney Center, she served as an Intake and Aftercare Specialist, Addictions Counselor, and Probation Officer, gaining extensive experience in care coordination, recovery support, and collaborative treatment planning.
Mrs. Lincoln-Ellzy earned a Bachelor’s Degree in Criminal Justice from Temple University. She approaches her work with a commitment to professionalism, collaboration, respect, and the continuing care process while supporting individuals on their journey of spiritual renewal, personal growth, and healthy reintegration.
