Discharge Planning
The eventual goal of outpatient treatment is to have the person reach a level of functioning that does not require regular therapy services. It is also recognized that some individuals require periodic maintenance therapy in order to maintain their optimal level of functioning. As such, each individual is assessed into one of two conditions:
- those requiring active treatment for which discharge or termination is a viable goal or,
- those who require prolonged maintenance.
These goals are listed on the treatment plan and are reviewed at the time of the treatment plan review.
