Community/ Diocesan Involvement
Monthly communication is maintained with each patient’s religious community or diocese. With the authorized consent from the patient, the superior or designee is informed of the assessment findings, overall plan of care, progress, issues related to return-to-ministry, and discharge planning. Whenever possible, face-to-face meetings are scheduled with the diocesan officials, community superiors, and team.
Upon discharge, a summary of the course of treatment and the recommendations for aftercare services, treatment, ministry, etc. are sent to the superior with the permission of the patient.
